We need to keep track
of the petitions that are out there, and keep you
posted on the signature gathering process. Please
fill out the following form to download the petition.

All completed petitions
should be returned as soon as possible. The county
offices will be counting signatures on a regular basis
so please do not hold completed petitions. All petitions,
even if they are not completely filled up, must be
postmarked by December 31, 2004.
